Radial Head Fracture: Symptoms, Treatment, and Recovery Time

A radial head fracture is a break at the top of the radius bone where it meets the elbow. It is the most common elbow fracture in adults and usually happens from falling onto an outstretched hand. Most are stable hairline fractures treated without surgery, and early gentle motion — not prolonged immobilization — is what protects your long-term elbow function.
If you have fallen and your elbow is painful, swollen, and hard to straighten or rotate, get it imaged. Radial head fractures are frequently missed on initial X-rays, and a missed one can leave you with permanent stiffness.

What Is the Radial Head?

The radius is the forearm bone on your thumb side. Its upper end, the radial head, is a disc-shaped surface that forms part of the elbow joint. It does two jobs: it articulates with the humerus so your elbow can bend and straighten, and it rotates within the joint so you can turn your palm up and down.
That rotation is why radial head fractures affect more than just elbow bending. Turning a doorknob, using a screwdriver, pouring from a pitcher, and accepting change from a cashier all depend on this small joint surface being smooth.

How Radial Head Fractures Happen

The classic mechanism is a fall onto an outstretched hand, sometimes abbreviated as FOOSH. When you catch yourself, force travels up the radius and drives the radial head into the humerus. The impact fractures the softer bone at the top of the radius.
Common causes:
  • Falls onto an outstretched arm, by far the most frequent cause
  • Direct impact to the elbow from a collision or blow
  • Sports injuries, particularly cycling, skiing, skating, football, and basketball
  • Motor vehicle accidents
  • Falls from height, including ladder and stair falls
  • Low-energy falls in patients with osteoporosis, where minimal force causes a fracture.
Women over 50 are at elevated risk, and radial head fractures are sometimes the first indication of undiagnosed osteoporosis.

Symptoms of a Radial Head Fracture

  • Pain on the outer side of the elbow, worse when you rotate your forearm.
  • Swelling around the elbow joint, often developing over hours
  • Difficulty straightening the elbow fully
  • Pain or a blocked sensation when turning your palm up or down
  • Bruising that may appear a day or two after the injury.
  • Tenderness directly over the bony point on the outside of the elbow
  • A grinding or catching feeling with movement
  • Reduced grip strength
Get urgent care if you have obvious deformity, numbness or tingling in the hand, fingers that look pale or feel cold, an open wound over the elbow, or pain that is severe and worsening. These can indicate nerve injury, vascular compromise, or a dislocation alongside the fracture.

How Radial Head Fractures Are Diagnosed

Diagnosis starts with a hands-on examination. Your physician will locate the point of maximum tenderness, test how far your elbow bends, straightens, and rotates, and check whether something is mechanically blocking motion versus whether pain alone is limiting it. That distinction directly affects treatment.
X-rays come next, and the views matter. Standard front and side views can miss a nondisplaced radial head fracture, which is why a dedicated radiocapitellar view is often added to profile the radial head clearly. If the fracture appears fragmented or the elbow feels unstable, a CT scan gives the detail needed to plan surgery.
Your wrist and shoulder get examined too. The same fall that fractures a radial head can injure the wrist joint or the ligaments along the forearm. Missing an associated injury is one of the more common reasons these fractures end badly.

Severity Grading: The Mason Classification

Orthopedic surgeons grade radial head fractures into three types. This is the single most useful thing to understand, because it determines both your treatment and your expected recovery.
Type I — Nondisplaced or minimally displaced
The fracture line is present, but the fragments have not shifted meaningfully, and nothing blocks the elbow from moving. These are the most common and are treated without surgery.
Type II — Displaced
A fragment has shifted, and it may mechanically block rotation or extension. Treatment depends on the degree of displacement, whether motion is blocked, and how much of the joint surface is involved. Some Type II fractures do well nonsurgically; others need fixation with small screws or a plate.
Type III — Comminuted
The radial head is broken into multiple fragments and cannot be reliably reconstructed. These require surgery, either fixation if the pieces allow it, or replacement of the radial head with an implant.
A fourth category is sometimes described for fractures accompanied by an elbow dislocation. When a radial head fracture occurs together with a coronoid fracture and ligament injury, it is called a “terrible triad” — an unstable pattern that needs prompt specialist care.

Treatment

Nonsurgical Treatment

Most Type I fractures and many Type II fractures are managed without surgery:
  • Brief immobilization in a sling or splint, typically days rather than weeks
  • Early gentle range-of-motion exercises, often started within the first week.
  • Ice and elevation for swelling
  • Anti-inflammatory medication as appropriate for you
  • Joint aspiration in selected cases, drawing off blood from inside the joint to relieve pressure and improve early motion
  • Physical or occupational therapy to restore extension and rotation
  • Follow-up X-rays to confirm the fragments have not shifted
The most important point about nonsurgical treatment is that it is not rest. The elbow is unusually prone to stiffness, and immobilizing it for weeks is one of the fastest ways to permanently lose motion. Early, controlled movement under supervision is what preserves function.

Surgical Treatment

Surgery is considered when fragments are significantly displaced, something mechanically blocks motion, the joint surface is disrupted, the elbow is unstable, or the fracture is part of a more complex injury pattern.
Options include:
  • Open reduction and internal fixation, realigning the fragments and holding them with small screws or a plate
  • Radial head replacement, implanting a metal prosthesis when the head cannot be reconstructed
  • Fragment excision, removing a small piece that is blocking motion
  • Ligament repair or reconstruction when the elbow is unstable
  • Radial head excision, occasionally appropriate in select older patients

Recovery Time and What to Expect

Nonsurgical recovery, Type I fractures
  • Week 1: sling for comfort, ice, early gentle motion begins
  • Weeks 2 to 3: out of the sling for most activities, motion improving, light daily use
  • Weeks 4 to 6: bone healing well underway, therapy progresses to strengthening
  • Weeks 6 to 8: most daily activities comfortable, driving typically resumed
  • Months 3 to 4: near-full function, return to sports and heavy lifting
  • Months 6 to 12: final gains in motion and strength
Surgical recovery
  • Weeks 1 to 2: protected motion begins, often within days of surgery
  • Weeks 2 to 6: progressive range-of-motion work
  • Weeks 6 to 12: strengthening added as healing allows
  • Months 3 to 6: return to work and sport depending on demands
  • Up to 12 months: continued improvement
One thing to expect honestly: many patients end up slightly short of full extension — often the last 5 to 10 degrees. It is common, usually not functionally limiting, and worth knowing in advance rather than discovering it at month four and assuming something went wrong.

Complications Worth Knowing About

  • Elbow stiffness, the most common issue, and the reason early motion matters
  • Post-traumatic arthritis if the joint surface healed irregularly
  • Heterotopic ossification, extra bone forming in the soft tissue
  • Nonunion or malunion
  • Nerve irritation, particularly of the nerve that wraps near the radial head
  • Persistent forearm rotation loss
  • Hardware irritation after fixation
Prompt, accurate diagnosis and a properly supervised motion program are what reduce these risks.

When to See an Orthopedic Specialist

See a specialist if you have elbow pain and swelling after a fall, if you cannot fully straighten or rotate your arm, if an urgent care visit produced normal X-rays but your symptoms persist beyond a week, if your motion has plateaued in therapy, or if you were told you have a “hairline” fracture but received no follow-up plan.
That second-to-last point deserves emphasis. Nondisplaced radial head fractures are among the more commonly missed injuries on initial imaging. If your elbow still hurts and won’t straighten two weeks after a fall, repeat imaging and a specialist examination are warranted regardless of what the first X-ray showed.

Frequently Asked Questions

How long does a radial head fracture take to heal?
Bone healing generally takes six to eight weeks. Functional recovery takes longer—most patients regain most motion and strength within three to four months, with continued gains for up to a year.
Do I need a cast for a radial head fracture?
Usually not. A sling or removable splint for a short period is more common, because prolonged rigid immobilization of the elbow causes stiffness. Your treatment plan should include a timeline for starting motion.
Can a radial head fracture heal on its own?
Nondisplaced fractures heal reliably without surgery, but “on its own” is misleading. They still require accurate diagnosis, a supervised motion program, and follow-up imaging to confirm the fragments have not shifted.
How long will I be out of work?
Desk-based work is often possible within one to two weeks. Work requiring lifting, gripping, or repetitive forearm rotation typically takes six to twelve weeks depending on fracture type and whether surgery was needed.
When can I drive again?
Most patients drive between two and six weeks, once they are off narcotic pain medication and have enough controlled motion to steer and react safely.
Can I still bend my elbow with a radial head fracture?
Often yes, which is exactly why these fractures get dismissed. Preserved bending does not rule out a fracture. Pain with forearm rotation and an inability to fully straighten are more telling signs.
Will I need physical therapy?
Most patients do. The elbow’s tendency toward stiffness makes supervised range-of-motion work the single most important part of recovery for both surgical and nonsurgical cases.
Is a radial head fracture the same as a broken elbow?
It is one specific type. “Broken elbow” can refer to fractures of the radial head, the olecranon, the distal humerus, or the coronoid. Which bone is involved changes the treatment entirely.
